WHAT FORMS ARE USED?

FORM 6
For inclusion of your name in the electoral rolls.

FORM 7
Can be filled by an enrolled voter who has an objection to inclusion of a name in the list. E.g. Death of a family member, if a member has moved out of residence etc.

FORM 8
For correction of any entry like age, gender etc by the voter. Those who are on the electoral rolls but do not have a voter’s i-card can fill this form for the same.

FORM 8A
To be filled when an enrolled voter changes his residence within the same assembly constituency. E.g. Change of address within Bandra.
